Long-term Disability Insurance
All full time employees of Hutcheson Medical scheduled to work at least 20 hours per week can enroll in the Long-term Disability plan with the following benefit. LTD plan is voluntary and is being insured by CIGNA.
Plan Details
- Eligibility: All full-time employees working a scheduled 20 hours per week.
- Monthly Benefit: Each employee can elect to increase the benefit to 60% of earnings to a maximum benefit of $6,000 per month.
- Elimination Period: Benefits begin the 181st day of disability.
- Benefit Duration: Benefits continue as long as employee is continuously disabled up to age 65. If disability occurs after age 60 benefits will continue per the ADEA schedule for a minimum of one year.
- Integration of Benefits: The benefit amount is reduced by other income payable for that same disability (Workers Compensation, other group disability insurance and employer or government retirement benefits, including Social Security). However, the minimum monthly benefit is $50.
- Maternity Benefits: Paid the same as for an illness.
- Mental Illness, Alcoholism or Drug Abuse benefit: A maximum of 24 months will be payable for disability, if you are not hospitalized.
- Survivor Benefits: If you are disabled for 180 days and a monthly benefit is payable at the time of your death, your survivor will receive 3 months benefit.
- Exclusions: Benefits are not paid to a disabled employee who is not under the care of a licensed physician, or whose disability is the result of a self inflicted injury or an act or hazard of declared or undeclared war. Disabilities caused by pre-existing conditions are not covered, unless (1) the insured is treatment free for 3 consecutive months on or after the effective date of coverage, or (2) the disability begins more than 12 months after the effective date of coverage, or of the disabled person has bee covered for 6 months with no treatment for the pre-existing illness.
- Pre-existing Condition: Is any condition, including pregnancy, for which medical advice, care, diagnosis or treatment was recommended or received, or for which prescription drugs were taken, within 12 months before coverage begins.
- Benefits are not taxable
- Definition of Disabled Employee: One who, because of an illness or injury, is unable to perform all the material duties of his/her regular occupation; and who, after receiving monthly benefits for 24 months, is unable to perform all the material duties of any occupation for which he/she may reasonably become qualified based on education, training or experience. An employee engaged in any occupation for pay or profit is not considered to be disabled.
- Insurer: The Long Term disability benefit is insured by Greater Georgia Life. Refer to the employee certificate for the exact provisions and limitations.
