Accidental Death and Dismemberment
The Voluntary AD&D plan is insured with The Hartford.
- Employees may choose from $50,000 to $250,000 of coverage in $50,000 increments on themselves not to exceed 15 times earnings..

Coverage may also be applied on the employee and family members. If Family is Elected:
- If Spouse Only – Spouse Benefit is 50% of Face Selected
- If Spouse and Children - Spouse Benefit is 45% & Children is 15%
- If Children Only – Children Benefit is 20%
- Spouses under age 70 are eligible for this coverage
- Children under age 19 or under age 25, if full-time student are eligible for this coverage
- Benefits are paid for Accidental Death and Dismemberment only
- Benefits are paid in addition to your Basic Group Life, Supplemental Group Life or any other life insurance you may have.
