Smoker rates apply if insured has used any tobacco products within the last 12 months. If the insured enrolls as a non-smoker and begins smoking during the year, employee must contact the Human Resources Department. The employee will be responsible for paying the full annual smoker rate premium. Falsification of insurance documents is a violation of personnel policy.
