Benefits at a Glance
- Each employee has the option of selecting increments of $10,000 to a maximum of $250,000
- During an employee's first 30 days of employment, coverage up to $250,000 can be issued without evidence of insurability.
- In subsequent Open Enrollment periods, employees may increase their coverage only $10,000 without evidence of insurability. Guaranteed Issue limits will be $250,000 (under age 60), $40,000 (age 60-69), $20,000 (age 70-79) and $1,000 (age 80 and over).
- Existing employees with no current Group Supplemental Life must provide evidence of insurability for any amount applied for during Open Enrollment periods.
- Coverage is designed to protect you while you are an employee of Coweta County School System and will terminate at retirement, termination of employment or eligibility, or discontinuance of the group policy in whole or in part.
- Includes an Accelerated Death Benefit which allows you to receive up to 75% in the event of a terminal illness, up to $150,000.. Terminally ill employees must have at least $10,000 of coverage.
- A Premium Waiver provision is included which will allow coverage to be continued without premium payment until age 70 or retirement, whichever is earlier, if an employee becomes totally and permanently disabled prior to age 60.
- Employees may convert their group supplemental life insurance coverage to a whole life individual policy issued by ING Reliastar.
- Employees may port coverage upon termination of active employment. Rates may differ from the rates for active employees.
- Amounts of coverage reduce with age as follows:
At Age 65 The benefit reduces to 65% of insurance in-force At Age 70 The benefit reduces to 40% of insurance in-force At Age 75 The benefit reduces to 25% of insurance in-force Monthly Premiums

Evidence of insurability is required for employees applying as "New Hires" for amounts in excess of $200,000, if under age 60; $40,000 if age 60-69; $20,000 if age 70-79; or $1,000 if age 80 and over. This benefit is insured by ING Reliastar. Refer to your certificate of coverage or the master policy contract for exact provisions and specifications.
